1. Information collected with the application
The assessor application collects legal name, former legal names when relevant, date of birth, current address, contact information, work history, credentials, availability, and supporting documents. It also asks only for a limited readiness response and, when volunteered, the name and date of a recent report that could be discussed later. This information supports identity matching, duplicate control, professional review, and preparation for a possible later screening process.
Do not place a Social Security number, driver-license number, passport, government identification copy, banking information, medical record, or full background report in the public application or an ordinary email. If any such information is lawfully needed, it will be requested later through the secure screening provider or another approved restricted process.
Optimize Enterprise will never call, text, or email asking you to send a Social Security number, banking information, password, medical or drug-test record, full consumer report, or copy of a government identity document. Treat any such request as a scam. Only an authorized screening provider may request necessary identifiers through its secure portal after a standalone disclosure and your written authorization.
2. The application records readiness—not a consumer report
An applicant may state that they can complete a separate screening if requested, that they have a recent report they can discuss if requested, or that they have questions before authorizing. If a recent report exists, the public application accepts only the provider name and approximate completion date. It does not accept the report itself.
Applicants should not purchase a report solely to submit an application. A report supplied by an applicant may be stale, incomplete, inappropriate for the later assignment, or subject to legal procedures that still must be followed. If screening becomes relevant, Optimize Enterprise initiates the approved process and keeps Social Security numbers, identity documents, and complete report contents with the restricted provider.
ID.me is an identity-verification service for participating organizations; it does not conduct employment background checks. It is therefore not presented here as a place to purchase or upload a background report. ID.me explains that distinction in its official support guidance.
4. Relevant, individualized review
Screening scope may vary with the proposed role, client, worksite, access, legal requirements, and risk. A check may include identity verification, employment or credential verification, and legally permitted public-record or criminal- history information. It is not used for advertising, resale, unrelated profiling, or public publication.
A screening result is considered with the nature and timing of the information, its relationship to the professional purpose, evidence of accuracy or rehabilitation, applicable law, and the rest of the record. The website does not promise automatic approval or automatic rejection based on a single result.
5. Confidentiality and storage boundaries
Date of birth, street address, former names, screening status, signature, and supporting documents remain in the restricted owner workflow and are excluded from the ordinary submission notification email. Access is limited to authorized people and service providers with a legitimate need. Reasonable administrative, technical, and physical safeguards and purpose-appropriate retention are used; no online system can guarantee absolute security.
The owner database stores only a provider or case reference, workflow status, and review date—not a Social Security number or a pasted copy of the complete consumer report. The screening provider remains the appropriate restricted source for the detailed report and identity-verification data.
6. Notice, correction, and adverse-action rights
When the Fair Credit Reporting Act or another law applies, the applicant receives the notices, report copy, rights summary, and opportunity to dispute inaccurate or incomplete information required before and after an adverse decision. Applicants may also ask Optimize Enterprise to correct application information or withdraw from the review process.
General federal information is available from the Consumer Financial Protection Bureau and the Federal Trade Commission. Applicable state and local protections may provide additional rights.
This notice describes the intended screening workflow. The stand-alone disclosure and authorization used for an actual report must be reviewed for the role, location, provider, and law in effect at that time.
